Visual Voicemail on Digium Phones

Does anyone have instructions (or link to instructions) on how to setup, install, and configure the visual voicemail app on D40/D70 phones with Asterisk? I’ve been scouring google and haven’t found anything but guides to use it once installed.

Due to some asterisk version limitations, I cannot use DPMA. Everything is configured with manual XML files (and is working fine). Regular dial-in voicemail is working fine. Visual voicemail would be nice to give my users.

The phone’s voicemail application is dependent on the use of DPMA. w/o it, your voicemail button on the phone is just capable of dialing an extension into Asterisk - typically you’ll map it to app_voicemail.
