Red Alarm on FX0/POTS Line

I’m having a issue where I can make calls fine across the POTS line plugged into the FX0. Inbound and outbound both work fine. At random times, I receive the following alarm and the port is locked until I unplug/replug the line into the card. I have a phone plugged into the pass-through port of the card and I have dial tone.
