Is there anyone who sets up the OS and Asterisk for a fee that I can either drive a box to or send them one?
I would prefer to drive it to them if they are located in North Carolina, South Carolina or Virginia.
But, I would be willing to ship it as well.

If you can manage to get a basic OS install (dead simple) there are LOTS of people who can do the rest remotely. All you need to do is install a basic linux package and SSHD so they can log in remotely. It works great!
The developers of Asterisk Management Portal can talk you through the basic install and remotely do the rest. They provide a great open source product and this is one way they are able to make some money indirectly.
I completely agree with mustardman. I would do it, but I’m not in US and I can’t install the OS (unless you have a KVM 
)
