The only HW you need is a linux box.

Yes. I’ve never tried using it on OSX, though, but apparently it works. It wouldn’t be any different, really, on OSX to what it’s like on linux - they’re both very similar unix-like operating systems.

Asterisk can run as a pure VoIP PBX using termination service providers, and then no extra hardware is needed.
If you want to terminate calls onto the PSTN locally, with analogue or digital, then additional hardware is required. Digium’s is the obvious choice, although there are some alternatives.
