PRI got event: Alarm (4) on Primary D-channel of span 1

I am having a daily issue of dropping calls. Here are a few of the logs I get.

Jan 13 11:09:45 NOTICE[12731] app_dial.c: Unable to create channel of type ‘Zap’ (cause 34 - Circuit/channel congestion)
Right now we are pointing at the telco and they are pointing back. Telco said that they never see the circuit drop or an OOS on their PRI port?
any thoughts, here is how the PRI is setup.

Now these are my possible settings for the LBO. I can increase the voltage but I don’t see any different.

0: 0 db (CSU) / 0-133 feet (DSX-1)

1: 133-266 feet (DSX-1)

2: 266-399 feet (DSX-1)

3: 399-533 feet (DSX-1)

4: 533-655 feet (DSX-1)

5: -7.5db (CSU)

6: -15db (CSU)

7: -22.5db (CSU)

any thoughts? any info is very much appreciated.
