Asterisk + PRI issue (detected RED alarm)

I have been having some problems with PRI and Asterisk, or maybe it is just me. It happens once maybe twice a day. I notice why it is happening, but I can’t seem to figure out a way to stop it from happening. I also notice that it is saying I don’t have a D channel defined. I am not sure why it is saying that either. After 5 or 6 seconds alarm cleared. Below are my details

If anyone has any suggestions/ideas it would be greatly appreciated.
