MOH insted of bridge for both partied on attended transfer

Hi,
Anyone here that can offer me proffesional asterisk support for one problem we have with callcentre sollution that uses Asterisk:

Caller A call to agent B. Than agent B makes outbound external call to caller C and make attended transfer.
And sporadically callers A and C are listening to MOH instead being connected. I see asterisk uses simple_bridge and i do not know why sometimes callers are not bridged.

Asterisk is 18.16 version
