Channel status is 'UNKNOWN"

I’m running Asterisk 1.4.4 and trying to put together a voice menu. Its very simple:

exten = i,1,Playback(invalid)

The menu works if I press a key during the playback of the message. However, it immediately hangs up at the end of the message instead of using the ‘t’ option.

Here’s the error message. Has anybody seen this?
