SIP configuration "Number is not in service"

I have a sip service with switch2voip.us/
I am using digest configuration on this page: http://switch2voip.us/index.php/customer-support/byod/sip-trunking-configuration

My Phone and IP Trunk are online and my IP Trunk is Registered.

Everytime I try to dial out I get this number is not in service no matter what number I call.

What am I missing?

You are missing the Asterisk version number and logs.

That message does not appear to be in the source code of Asterisk 1.8, so it is presumably coming from either the remote system or from some third party dialplan code.
