Outgoing FWD IAX2

I’ve been struggling with outgoing IAX2 calls through FWD. The calls always fail with
Dial failed due to CHANUNAVAIL.

The system is Asterisk@home 2.1, Asterisk 1.2.0, TDM400P card.
When an outgoing test call to 613 is made the asterisk window displays-
– Executing GotoIf(“Zap/1-1”, “0?6”) in new stack
– Executing SetCallerID(“Zap/1-1”, “Console”) in new stack
– Executing NoOp(“Zap/1-1”, “CallerID set to Console”) in new stack
– Executing SetGroup(“Zap/1-1”, “OUT_2”) in new stack
– Executing CheckGroup(“Zap/1-1”, “2”) in new stack
– Executing SetVar(“Zap/1-1”, “DIAL_NUMBER=613”) in new stack
– Executing SetVar(“Zap/1-1”, “DIAL_TRUNK=2”) in new stack
– Executing AGI(“Zap/1-1”, “fixlocalprefix”) in new stack
– Launched AGI Script /var/lib/asterisk/agi-bin/fixlocalprefix
fixlocalprefix: Could not parse /etc/asterisk/localprefixes.conf
– AGI Script fixlocalprefix completed, returning 0
– Executing SetVar(“Zap/1-1”, “OUTNUM=613”) in new stack

Can anyone offer a suggestion?
