[HELP] Directory hang-ups

Running version 1.2.7.1, FreePBX, Asterisk@Home.

We have the extension directory set up, however once you enter the first three digits of the person’s last name and press “1” to confim, the system hangs up on the caller.

Has anyone seen this before, and if so, any hints at resolution?
