Sometimes tdm410 block fxs/fxo ports

Sometimes fxs or fxo ports of my tdm410 card, are blocked.
Then I had to restart my system.
I can hear from the phone connected to the fxs port a busytone (and I can’t stop it with hookflash) or a mute tone…
So I saved the cli output.
Note that nobody answer… but there was an automatic hangup+starting simple switch sequence… and it repeats a lots of times…
Could someone help me to know what happen…?

If it could be useful:
It happened on CentOS 5.4 with both dahdi 2.2.1 and 2.2.0.2 and several version of Asterisk (1.6.1.17 included)
