Good VoIP Provider Needed ASAP

I need to find another VoIP provider ASAP who can provide the following:

  1. EXCELLENT sound quality on calls.
  2. Flat rate for unlimited inbound calls.
  3. Multiple channels that can be shared across a range of DIDs to allow many simultaneous inbound calls.
  4. DIDs from a variety of US cities (and preferably international numbers too).
  5. MUST support SIP and GSM.

Any recommendations would be greatly appreciated.
