Starting simple switch / Hungup

I have been beating my head against a wall for a while with this issue, and have found nothing online except for a couple others with the same issue and no answers…

We have two d4 AMI T1’s connected to the asterisk box. One is for inbound/outbound local calls and the other is purely outbound for long distance.

The local T works perfectly, however whenever I plug in the LD T, i get:
– Starting simple switch on ‘Zap/25-1’
– Starting simple switch on ‘Zap/26-1’
– Starting simple switch on ‘Zap/27-1’
– Starting simple switch on ‘Zap/28-1’

– Starting simple switch on ‘Zap/48-1’

This is then followed by:

[Jul 29 23:34:42] WARNING[19586]: chan_zap.c:5654 ss_thread: getdtmf on channel 25: Success
– Hungup ‘Zap/25-1’
[Jul 29 23:34:42] WARNING[19587]: chan_zap.c:5654 ss_thread: getdtmf on channel 26: Success
– Hungup ‘Zap/26-1’
[Jul 29 23:34:42] WARNING[19588]: chan_zap.c:5654 ss_thread: getdtmf on channel 27: Success
– Hungup ‘Zap/27-1’

on all the channels

If there is an active call, it will get disconnected when this occurs.

This repeats again and again with no end in sight.

Any ideas?

What happens if you swap the T’s?

Same result… just different channel segment. I double checked the T1, and it is coming clean from the telco. It is also working fine in our old phone system. Right now I have changed gears a bit and have connected a PRI T1 that we were not currently using. Hopefully the same problems do not occur. In any case, I would love to figure out why the d4 AMI LD line was not playing nice.
