Demo-Congrats Message is Playing Randomly

This morning I received a call from a client saying when people are calling in they receive the proper IVR message, but when they press 3 to get to a receptionist the following message plays:

I’ve made no changes to the system. This is the second time it has happened and all I can do to fix it is create another queue. I am running PBX in a Flash 1.3 on an Intel Atom board. Here is exactly what is happening:

To fix I created a new queue with the exact settings of the last queue. The queue is set to dial a receptionist (ext. 100-102). This did the job, but why did this happen and how do I stop it. No where do I have it set to dial the ‘demo-congrats’ message.
