Call is being forwarded with its own number in the To header

Hello,

I’m running 1.8.26.1 using Feee PBX GUI. I placed an inbound call and after 5 seconds, the call is forwarded back out to the network with the PBXs number in the To header, creating a loop. Call forward is not setup on the Polycom phone or at the Free PBX GUI. Follow me is disabled. Thoughts?
