I would like to know if anyone is using asterisk for a voicemail option.
Looking to have a 20,000+ seat voicemail system.

I have set up asterisk as a voice mail system but not for 20k plus users. You will probably need to span it across multiple servers and have the VM’s stored in a DB (which I believe can be done although I never did it myself).

Maybe not storing the sound files on a DB but having the voicemail backend on a DB - storing the information about the voicemail only.
